Our Policies

APPOINTMENTS
We try our best to stay on schedule. Please help us by being on time for your appointments. If you have difficulty keeping scheduled appointments, we will be happy to place you on our call list for last minute fill-ins.

CANCELLATIONS
We reserve time especially for you. If you are unable to keep your appointment, please call us at least 48 hours prior to your appointment so that other patients needing help may be seen. Due to the length of these appointments, there will be a charge for surgical and initial therapy appointments broken without 48 hours notice.

FEES
Our fees reflect all the knowledge, skill, and time required to provide the service and will be discussed fully with you beforehand. Payment is due at the time services are rendered. If other arrangements are necessary, this should be discussed prior to your appointment. A service charge will be charged to your account for all returned checks. All charges incurred for the initial exam are due in full on the day the services are rendered. We ask that the patient pay for the initial visit and if you have insurance benefits, insurance will reimburse you. AT subsequently appointments you will only be responsible for your insurance co-payment.

INSURANCE
Each patient is responsible for his/her own account. We will gladly assist you in submitting information to your insurance company. It is your responsibility to furnish complete information including plan books, claim forms, and telephone numbers. The dental insurance plan you are covered by is a contractual agreement between your employer, yourself, and the dental insurance carrier. We cannot accept responsibility for collecting your insurance claim or for negotiating a settlement of a disputed claim. Please remember that the insurance companies reimburse the patient for the fees paid to the doctor and usually only pays a percentage of the charges. Therefore, it is your responsibility to pay any deductible, coinsurance or any other balance not covered by your insurance company at the time services are rendered. After 60 days following treatment, if there has been no payment or only partial payment issued on a claim, you will be responsible for the balance of your account at the time. With two insurance carriers (double coverage), we accept the co-payment from the patient and file a claim for the balance due with the patients insurance company. After this claim is paid to our office, we will the file the secondary insurance and assign the benefits to you.
